Nursing home costs can be one of the largest expenses seniors face, but many of these costs may be tax-deductible.
If the stay is primarily for medical care, you may be able to deduct some or all of the expenses as medical expenses.
Nursing home expenses are claimed on Schedule A (Form 1040) and reported on IRS Form 1040 under rules set by the Internal Revenue Service.
Here’s how the deduction works.
When Are Nursing Home Costs Deductible?
Nursing home expenses are generally deductible if:

- The primary reason for being in the facility is medical care
- A licensed healthcare provider certifies the medical need
- The individual requires substantial assistance
If the primary purpose is medical care, the entire cost — including meals and lodging — may qualify as a medical expense.
Example: If a doctor certifies that a senior requires full-time nursing supervision due to a chronic illness, the total annual nursing home cost may qualify as a deductible medical expense.
Medical Necessity Requirement
To qualify, the resident typically must:
- Be unable to perform at least two activities of daily living without assistance, or
- Require supervision due to cognitive impairment
Activities of daily living include:
- Bathing
- Dressing
- Eating
- Transferring
- Toileting
- Continence
Medical documentation is essential.
What If the Stay Is Not Primarily Medical?
If a nursing facility stay is primarily for personal or custodial reasons rather than medical care, only the portion attributable to medical services may be deductible.
The facility may provide an annual statement showing the breakdown between medical and non-medical costs.
Example: If annual nursing home costs total $80,000 and the medical portion is $60,000, only the $60,000 may qualify as a medical expense.
The 7.5% AGI Threshold
Nursing home expenses fall under medical expense rules.
You may deduct medical expenses only to the extent they exceed 7.5% of adjusted gross income.
Only the amount above that threshold is deductible.
Example:
If your adjusted gross income is $90,000, 7.5% equals $6,750.
If nursing home costs total $75,000, you may deduct $68,250.
You must itemize deductions to claim the benefit.
Paying for a Parent’s Nursing Home
If you pay nursing home expenses for a dependent parent, those costs may also qualify as medical expenses.
To qualify:
- The parent must meet IRS dependency rules
- You must provide more than half of their financial support
This can provide significant tax relief for caregivers.
Example: If you pay $50,000 for a dependent parent’s nursing home care, those expenses may count toward your medical deductions.
Long-Term Care Insurance and Nursing Homes
If long-term care insurance reimburses part of the nursing home cost, only the unreimbursed portion is deductible.
Premiums for qualified long-term care insurance may also be deductible, subject to age-based limits.
What Does Not Qualify?
Generally not deductible:
- Purely residential senior living without medical care
- Independent living facility rent
- Personal expenses unrelated to medical treatment
Proper documentation is critical.
Recordkeeping Tips
Keep:
- Doctor’s certification of medical necessity
- Annual billing statements
- Proof of payment
- Insurance reimbursement records
Clear documentation supports your deduction in case of IRS review.
